Before you arrive

Your oncologist will have given you an information pack at the time of your first consultation.  This pack contains most of the patient information on this website that relates to you personally.  Included in the pack are some forms for you to fill out and bring to your first (Treatment Planning) appointment at ARO.

Please sign your Consent form in conjunction, and following discussion with, your oncologist. This confirms you have received all the information you require in order to make an informed choice and have agreed to your treatment. The other forms can be downloaded from this website if required. Please fill out these forms and bring with you to your Treatment Planning appointment.

Insurance and Prior Approval

Please also discuss your insurance position with your oncologist.  Your oncologist will send through a booking form to ARO with the information we require to go forward to the next step in your treatment process.  This includes obtaining prior approval from your insurance company.  In the event you are required to pay a deductible or co-pay for your treatment, you will be notified before your Treatment Planning appointment.
